If I observe any unusual hazard during participation, I will remove myself from participation and bring such to the attention of the nearest official. 3. I am a willing participant at the Gold Medal Squared Volleyball Clinic and release and hold harmless Gold Medal Squared and their officers, officials, and employees. 4. I certify that I am in good health and able to participate in the program activities. If I do have a special medical condition, I will bring it to the attention of the clinic staff.
